Paciente de 39 años que presenta placas eritematosas con exudado y fuertes dolores poliarticulares, diagnosticado de Síndrome de Gardner Diamond los dolores no ceden con los analgésicos y AINEs habituales y empieza a tomar opioides sintéticos y termina haciendo un cuadro por dependencia a opiáceos.
En ese momento requieren apoyo de la Unidad de Conductas adictivas para después de hacer una desintoxicación hospitalaria mantener la abstinencia y realizar un seguimiento posterior de la adicción. No obstante el cuadro de dolor vuelve a repetirse y los hematomas y exudados se exacerban en determinados momentos. Siendo difícil su control.
Pretendemos, en este artículo, hacer una descripción del cuadro clínico, valorar la patología concomitante al síndrome de Gardner Diamond y hacer un mantenimiento de la abstinencia analgésicos y un control del cuadro psíquico, dermatológico y del dolor, evitando de nuevo un abuso de opiáceos.
El objetivo del artículo es hacer una revisión, seguimiento y evaluación de este caso para intentar dar una respuesta a un síndrome con difícil resolución terapéutica con tendencia a la iatrogenia y a la automedicación.
El tratamiento posterior con fármacos reguladores del estado de ánimo, antipsicóticos y apoyo psicoterapéutico, disminuye el dolor, espacia los brotes y disminuye la necesidad de analgésicos, mejorando la calidad de vida del paciente.

Abstract
A 39 years old patient who present eritematose’s patch and hard poliarticulation pain, diagnosed with a Gardner Diamond syndrome has pain which does not stop with usual analgesic and AINEs (no steroid anti-inflammatory). He begins to take opioids and he develops dependence.
Nowadays he needs the support of UCA (Unidad de Conductas Adictivas) and he has been detoxified in a hospital, monitoring the addiction. However, the pain comes back with hematomas and exudes sometimes. With big controlling difficulties.
The article wants to make a revision, keeping a record and evaluation of this case trying to give an answer to this syndrome with a complicate therapeutic solution with a big tendency to iatrogenia and self-medication.
The rear treatment with regulator medicines for mood, antiseptics and psychotherapist support, makes the pain goes down, spreads outbreaks and reduces the painkiller needs, improving the quality of patient life.
