Clásicamente se ha asociado el consumo de cigarrillos con la ansiedad y el estrés. En esta revisión se analiza el estado actual de esta cuestión. En los estudios epidemiológicos se encuentra una clara y significativa asociación entre fumar y distintos trastornos de ansiedad. Igualmente, la mayoría de los estudios encuentran que los fumadores tienen una ansiedad rasgo mayor que los no fumadores o los que fuman pocos cigarrillos. Esta relación se encuentra tanto en adolescentes como en adultos. Fumar incrementa el estrés. Al dejar de fumar se reduce el nivel de estrés y la ansiedad, relación que se incrementa con el tiempo de abstinencia. Al tiempo, el estrés es un elemento que impide a muchas personas dejar de fumar o un facilitador de la recaída. Con la evidencia disponible se analiza el papel que esta relación tiene para la prevención y el tratamiento del tabaquismo.
